The website’s consultation, referral, career, and newsletter forms send the information you enter to Private Care Duty, LLC through a third-party form delivery provider. Career applicants may attach a résumé, professional license, and relevant training credentials. Please do not include diagnoses, medications, insurance numbers, Social Security numbers, financial information, or other sensitive medical or identification information. Completed I-9 forms and identity documents are collected separately through a secure post-selection onboarding process.

How information is used

Information you submit may be used to respond to your request, discuss services, follow up on a referral, schedule a consultation, consider an employment application, or provide requested care guidance.

Website technology

This website may use standard hosting logs needed for security, reliability, and performance. It does not use an account system or store form submissions in your browser. The form delivery provider may process and temporarily retain submissions as part of delivering them to our inbox.
